The aim of the chapter is to explore about Diffuse steatonecrosis by developing a case of acute abdomen with palpable lump. Diffuse steatonecrosis secondary to acute / gangrenous pancreatitis identifying as vague abdominal mass has diagnosis problems. A special method/ techniques are important for demonstration of the fatty acids crystalloids. This chapter developed bases on a case of acute absomen with palpable lump. We report a case of 22 year old pregnant female, admitted in the emergency ward with complains of acute abdominal pain and lump abdomen umbilical region since 15 days was selected for developing this study. On palpation, abdomen was soft, non-tender with a large irregular lump felt in the para-umbilical region. It can be concluded that a large thick jumbled up omental mass was developed on the emergency exploratory laprotomy. The mass was adhered to parietal wall along with complete pancreatic necrosis. Diffuse steatonecrosis should be considered in the differential diagnosis in the context of such clinical setting.
