We present imaging findings of an Quadricuspid aortic valve (QAV). Echocardiographic findings were supplemented with other diagnostic procedures. QAV is a rare congenital heart disease. The functional status of QAV is predominantly a pure aortic regurgitation. Clinical manifestations of patients with a QAV depend on the functional status of the QAV and the associated disorders. A 70-year-old female patient was suffering from progressive shortness of breath due to severe aortic regurgitation (AR), followed approximately two years prior to surgery. Surgery went uneventful, and she was released home on the fifth postoperative day. Good postoperative result in a follow-up some four months later to intervention has been established.
